Please fill out this form to register for our after school program. You must register your child again for each new school year. Return the form by August 5th or before you plan to have your child attend SEAS. The registration fee will be billed through FACTS.

*Additional Authorized Pick-up name and phone numbers ensure someone other than the parent/guardian can pick up your child. Be advised that individuals other than the parent/guardian are required to show proper picture ID at time of pick up.

*A note should be provided and given to the teacher if you plan for your child to stay on a Drop In basis. If an unexpected situation arises during the school day, call the school office so the teacher will know to send your child to SEAS. Be sure that the person is listed on your authorized pick up list.
